About Us
White Plains Hospital Physician Associates (WPPA) is a division of White Plains Hospital, developed with the intent of aligning local practicing physicians with Hospital services. As a result, our patients can benefit from greater access to an expanding breadth of outpatient specialty services close to home. By enabling physicians and the Hospital to work more closely than ever before, we ensure the best possible outcome for each individual patient by sharing information more directly, promoting growth, utilizing resources efficiently and effectively, and developing quality care to its fullest.
As a key member of the Acute Care Transition Team (ACTT), you will contact patients within 48-hours of discharge from the acute care setting with the goal of a 100% contact rate. You will work to answer general and specific questions in regards to the patient’s medical encounter, plan of care, potential clinical complications, and/or discharge instructions. The successful candidates will work with our other team members to bridge the divide from the outpatient setting to the hospital as well as from hospital to home to support the acute care staff and their family members as they navigate the healthcare system at White Plains Hospital and our affiliates. The professionals we are seeking will also help to facilitate two-way communication between the acute clinical setting and the home environment which includes encounters from the emergency department, inpatient setting, observation, labor & delivery, and/or ambulatory surgery.
Education & Experience
- Current New York State Registration as a Physician Assistant or Family Nurse Practitioner
- 5 years of experience as a nurse in an appropriate clinical setting with inpatient, emergency department, or care coordination experience preferred
- Heavy phone use
- Extensive Computer usage
- Ability to respond to frequent calls while using a professional and courteous demeanor
- Ability to remain in stationary position 90% of time; and OR will also be able to move about inside the office and within the hospital.
